Mod works on only Normal & Elite stages.
Every final stage (10th) normal stage, the mod won't work initially but after lot of tries or improving your power level significantly, it works. I don't really know what makes it or not make it work on the final stage (10th) of normal maps. Same goes for the last stage of Elite stages as well.
It also works on some event bosses but not where multi-users are involved.
On Trial - I am not really sure if the mod is required or not. If you have stamina, you can farm trials all day.
Multi-user, Arena, Guild & Wild boss - I hope it worked here somewhere but it doesn't.

Observed that if you have 70-75% of the required BP in the 10th stage (boss) of normal stage, then the extreme damage works for those stages otherwise it won't.
