It's basically SFW. Watching ads for this game will make it NSFW.
However, I don't know of a way to always make it NSFW.

Whether you have SFW or NSFW is determine upon initial install. If you get SFW uninstall and try something different. Supposedly if you follow a link from appbrain.com it will always be NSFW so try following that link to install. It's possible challenge mode is disabled for SFW version to keep it SFW (I know other games have done that.)

A method for using the above link to get NSFW mod
1. In app settings clear cache/data for SFW version
2. Uninstall SFW version
3. Open link from AppBrain
4. Keep store page open and switch to file explorer
5. Install mod

if is SFW try searching for other methods. This is the only one I know. Good luck
